In agreement with the clinic's gynecologists, all routine examinations and ultrasounds, such as the 1st and 2nd trimester ultrasounds, are part of the follow-up, or the search for albumin in the urine, are included in the follow-up.

Follow-up by our gynecologists

It generally includes 6 appointments per pregnancy during which the well-being of the fetus and the mother are assessed. In addition, the risk of infection, the risk of gestational diabetes, rhesus constellation anemia, etc. will be assessed.

 

1st trimester screening

First trimester screening is part of screening for trisomies (13, 18, 21) and is carried out from the 11th week + 0 and 13 + 6. It is important to know that these anomalies are relatively rare. Trisomy 21, for example, is found in one in 500 pregnancies.

This screening combines the risk related to the patient's age, the thickness of the fetal neck and the blood level of two hormones taken from the mother: PAPPA and HCG. Ultrasound allows the study of different organs: the head, limbs, abdomen, kidneys, heart and, moreover, to detect morphological anomalies linked or not to chromosomal anomalies (holoprosencephaly, cardiac anomaly, laparoschisis, etc.).

If serum markers show values outside the norm, this may be correlated with an increased risk of Down syndrome. If the calculated risk is low, the origin of this variation may be something else.

Ultrasound at this stage also allows in cases of twin pregnancies to define chorionicity (true or false twins).

 

2nd trimester screening

Second trimester screening is performed between 14 and 21 weeks and allows the detection of neural tube closure defects (Spina Bifida, exencephaly) by measuring AFP (Alpha Fetoprotein) in the blood.

If the first trimester screening could not be carried out, it can be made up for by screening for trisomy 21 by blood measurement of AFP, estriol or beta HCG.

 

Morphological ultrasound of the 2nd trimester

It is ideally performed between 21 and 24 weeks. During this examination, each organ is studied in detail as well as its growth. The placenta and the cord are also studied. This ultrasound allows the screening of morphological abnormalities (cleft lip, cardiac abnormalities, etc.). The ultrasound machine used in our unit, with which we perform 3D and 4D ultrasounds on medical indication, is one of the most modern in the world, it allows for a more precise diagnosis. If conditions allow, a brief time of the examination may be devoted to a 3D ultrasound during which you may have the chance to see your fetus move, smile or suck its thumb.

 

Induced hyperglycemia

Furthermore, pregnancy-related diabetes can occur in a non-diabetic patient. This is why an oral glucose tolerance test (OGTT) is performed between the 24th and 28th week. During this test, we measure blood sugar levels before, then one hour and two hours after, the oral intake of 75g of glucose. Most of the time, this diabetes resolves with diet, but sometimes temporary insulin therapy may be necessary. This diabetes very often disappears after pregnancy.
